Hi, looks like you submitted your application using the Rich Text tab, which means the HTML isn't working. Please copy your entire application and paste it into the HTML tab on the Edit Entry screen.
It looks like you also have some stray line breaks throughout the entry -- please edit those out as well.
And I know you are already planning to add the answer to question #5.
